Changing your phone number or email address

You will need to show us identification before we can change your phone number or email address.

You will need to show us proof of your date of birth if it needs changing on your medical record.

It is helpful for us to have evidence if you have officially changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate. However, if you have not legally changed your name, we can still amend this on your medical record.
