Registration
The practice welcomes new patients.
To register with the Wellbrook Medical Centre we would ideally like confirmation of who are and where you live. All newly registered patients over the age of 14 will be asked to complete a health questionnaire.
We would like some form of ID such as a passport, driving licence or bank card along with something that shows your home address. If you do not have an ID or confirmation of your home address, don't worry but please mention this to the Receptionist when dropping off your registration forms.

You may print off a registration form, Patient Questionnaire & Summary Care Record form here, please fill them out and bring them with you to the practice along with your ID and address details.

Addtional Information
If you have changed your name you must have a supporting document regarding the change or name, eg marriage certificate, Decree Absolutle or Deed Poll Document.
