Accessing your Health Record

All patients have online access to their full medical record from 1st February 2023 onwards. You can access your records online using your NHS account (using the website or NHS App) or by registering for SystmOnline. If you would like to see any information added before this date, you can request online access to your full medical record. Please read the leaflet below and complete the request form and drop it in to Reception:

You can also get a paper copy of your medical records for free. Please put your request in writing to the surgery and allow one calendar month for your notes to be ready for collection.
